Pursuant to Article 6(1)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), I hereby agree to the processing of the personal data contained in the agreement and the submitted documents for the purpose of handling the contact via the electronic contact form. *

Using the contact form and submitting any personal data in it, I hereby acknowledge that:

1. the data controller is Karton-Pak Spółka Akcyjna, ul. Hutnicza 10-12, 67-100 Nowa Sól, Poland;
2. providing data is voluntary; however, in order to receive feedback, it is obligatory;
3. the purpose of data processing is to make contact via the form available on the controller’s website;
4. the data are received by the controller;
5. the data will not be transferred to any third country or international organisations;
6. the data will be retained for the period necessary to maintain the contact and, after its termination, (unless further cooperation is established) for the period of 10 years;
7. I have the right to access data, request to rectify them, delete them, restrict their processing;
8. I have the right to withdraw my consent to the processing of personal data at any time; the withdrawal of consent to the processing of personal data in accordance with Article 7(3) of the said Regulation does not affect the lawfulness of processing based on consent before its withdrawal;
9. I have the right to file a complaint with the Personal Data Protection Office;
10. the data will not be profiled;
11. I have the right to object to the processing of personal data. *

Pursuant to Article 4(11) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), I hereby give my consent, absolutely voluntarily and consciously, having a real and free choice, to the processing of my personal data in order to establish a business contact. The controller of obtained personal data is Karton-Pak Spółka Akcyjna, ul. Hutnicza 10-12, 67-100 Nowa Sól, Poland, KRS No. 0000175333. *

I hereby acknowledge that I have been informed that:

1. the data will be received by all entities involved in the achievement of the purpose for which the data have been submitted, and whose participation is necessary to achieve the purpose or which will participate in the achievement of the purpose regardless of my will or the Controller’s will; such entities include, in particular, courts and other public authorities; 
2. the controller does not intend to transfer data to third countries and international organisations;
3. the period for which the data will be processed depends on the purpose for which they are processed; when the purpose has been achieved, the data will be processed for the period of further 10 years or possibly for a longer period if the applicable law provides for the possibility of initiating any proceedings with respect to the data that have been processed by the controller;
4. I have the full right to access data, request to rectify them, delete them, restrict their processing;
5. I have the right to object to the processing of data;
6. I have the right to file a complaint with the supervisory authority, i.e. the Personal Data Protection Office;
7. providing data is mandatory to enter into an agreement with the controller and, as far as proceedings before public authorities are concerned, the obligation to provide them results from law; the refusal to provide personal data is an obstacle preventing the implementation of the agreement and the achievement of the purpose for which the data are processed;
8. the data will not be profiled;
9. I have the right to withdraw my consent to the processing of personal data at any time; the withdrawal of consent to the processing of personal data in accordance with Article 7(3) of the said Regulation does not affect the lawfulness of processing based on consent before its withdrawal. *
